(Moorhead, MN) — A Moorhead assisted living facility is being fined by the state Department of Health for the second time this year. Farmstead Care received a one-thousand-dollar fine in March for conditions that led to a resident falling and suffering injuries, including two brain bleeds. The facility was inspected last month and additional problems were found. The facility has been fined another seven-thousand dollars and has until early September to make needed improvements.
